Grant recipient final report information

All grantees are required to submit a report once the grant funds have been expended for their intended purposes. Recipients of a one-year grant must submit this report within 365 days after their grant request is approved. 

The Foundation introduced a revised form to be used as both an application and a final report for grants that are approved.  Please read the instructions and download the following MS Word document. Use the tab key to move from one form field to another.

Grantees who fail to meet reporting requirements are ineligible for additional grants from the Community Foundation until the appropriate reports are submitted.
